I'd like to suggest you reset your level of expectation.
As you've already discovered, you can use miles to upgrade.
If you have no more miles, then you can either pay for premium class or buy-up (invariably for a hefty fee) or when you reach a high level of status (GS/1K on UA or HON/SEN on LH) use a status-linked instrument (e.g. UA GPU on UA/LH).
Those are the extent of your options: there are no free upgrades on international travel (unless you get very lucky with a day-of operational upgrade).
