I was pretty shocked to hear that he 'forgot' that he already had this card, which he applied for maybe six years ago. It's still active. Is there any way for him to close the card, and re-apply to get the introductory offer?

He can call Chase to see if there are any offers on his account. That is their best BT card, they usually offer promotional balance transfers periodically. Penfed has a good balance transfer card, low apr and an introductory 0% fee for transfers.
It goes without saying though, there is not much point to doing a balance transfer unless he is willing to cut down on his spending or stop using some of the other cards, a balance transfer away from cards that are normally used is a good way to get deeper in debt.
