I have processed payments from abroad (Asia and Europe). If the sender is an ex-pat this can easily be done. If a nonUS citizen, it is trickier. Funds can be recycled using several options.
As to US Addresses, I believe the Virgin of Australia as well as many other tricksies taught us we can live in many places at once.

Opening AP and GW accounts is impossible for non-US residents I think. Both require SSN, US address and US bank account in addition to US issued CC.
