From the TSA's "Special Needs Memo" (dated September 25, 2006), which is no longer found on their website, amongst the medical liquids permitted are:
liquids (to include water, juice, or liquid nutrition) or gels for passengers with a disability or medical condition;
Regarding the specific medical conditions:
It is recommended (not required) that passengers bring along any supporting documentation (ID cards, letter from doctor, etc.) regarding their medication needs.
There's no requirement to disclose or even discuss the specific medical conditions that require any medical liquids. Furthermore, on a practical level it's a violation of privacy; on a legal level, I suppose since they're federal employees it could be construed as a violation of Title II of HIPAA for them to discuss it with the traveler.
