Just received one of these for a flight to YUL that I took late last week. Took me 15 minutes or so to complete (lots of space for free format text as well as the multiple choice type format) and it asked further questions based on my first responses.

Given that the flight was disappointing (as was my visit to the Flounge ) it will be interesting to see if there is any further contact as a result of my (very specific) feedback...time will tell.

Didn't complete an on board survey and haven't e-mailed a complaint (only returned to UK today via Schiphol) so this seems to be random and not based on previous contact.

Hope they take notice of my comments (but I wont be holding my breath...)
