There was a write-up on the subject in Inside Flyer a while back, I think that the only airline offering a top level lifetime elite is TWA.

The Continental Infinite Elite was offered to those who met the published requirements for Gold Elite for five consecutive years, and was last listed in the OnePass literature in 1993. At the time of the offering the life expectancy of the airline was much less then the majority of it's passengers!
