Mine (B1/B2) doesn't, issued 02/2005 - though things have clearly changed since then.

[Pause to reminisce - my first US visa entitled me to make 'unlimited applications for entry' and was supposed to last as long as I did. I have never needed an ESTA.]

I'd never heard of an I-797, so what's an I-797?

I searched and found there are several versions of I-797, and apparently what we're taling about is an I-797A.
