Mine (B1/B2) doesn't, issued 02/2005 - though things have clearly changed since then.

I'd never heard of an I-797, so what's an I-797?

I searched and found there are several versions of I-797, and apparently what we're taling about is an I-797A.

Do I need to complete a form? There seems to be no way of applying for one.
