I called the AE SE desk and it's as I speculated. You can make as many changes as you want in that phone call, and the $90 fee is waived on your first call. As an aside, the agent also told me that if you had 6 passengers on your booking, you could change all 6 at once and not pay a change fee (the first time you call).
