I was surprised to find quite a few vendors on Isla Barú's Playa Blanca when I was there several years back. As tourism in that area only expands, I imagine the vendor situation is even worse today. Isn't the land connection to Isla (as Johnny points out, technically a peninsula and not an island at all) Barú now paved? That certainly can't help matters...
Want a good island trip from Cartagena? Hop on Copa's flight to San Andrés!

Edited to add: I remember people suggesting that it's possible to avoid the scrum of tourists and shady tour operators at the main Muelle Turistico and instead hire fishermen's launches privately somewhere in Laguito? Anyone ever done this?
