Through my business I spend on average $40,000 - $50,000 per month on my United Club Card. When I fly international, which is several times a year, I always fly first or business class so I have never needed elite status. I also try to always fly partners such as ANA, Asiana, Lufthansa, Thai or Singapore Air which makes for a much nicer experience than flying United.
For short flights I will sometimes book economy, and in those cases the Club Card still gives most of the benefits of having elite status, including two free checked bags and of course lounge access.
I haven't had elite status with any airline in many years and have not missed it at all. I would recommend using the United Club Card and earning 1.5 miles per dollar and not worry about elite status.
