How do they know if I filled out an ESTA?
Something struck me

My last trip to the US, although I filled out and printed the ESTA approval, no one at check in at LHR asked to see it.

So how would they otherwise know if I have an ESTA? Isn't there a risk of a pax boarding, arriving at passport control in the US only to be returned back to LHR for not having one, at BA's cost?
