It means you get one bag above the standard allowance. If the country/region you're visiting isn't listed under the _standard_ baggage terms, the standard allowance is 2x 50lb bags, and then you'd get an additional free 50lb bag on top of that. There are countries listed on the standard baggage terms, but not on the medallion baggage terms (e.g. intra asian flights) where you'd only get 1x 50lb bag free as the standard allowance, plus an additional medallion 50lb bag free as well.
