AT&T will probably be your only choice in more remote areas. If all you care about is 911, you don't need a SIM at all; the law requires any carrier to accept emergency calls from any phone.

Refills are available everywhere, but I think SIM cards are mostly available in mobile phone shops and some retailers like Walmart and Target. Not 100% sure of that, though - I've never bought a prepaid SIM in the USA.
