Intl SOS do a good job, IMO. No idea if they will cover Iran, but I've seen them cover some pretty unusual situations so they're probably worth trying.

Quoted them too. They charge $1500 for a month for Iran, compared to $189 for a 'normal country'. I'm simply not willing to pay that much (for nothing).
