You could buy a medical policy from another country which doesn't have an issue with Iran? I am thinking in a similar way to how Americans who travel to Cuba must buy travel insurance from another country (say, Canada) as Cuba insists every visitor has it, and US policies won't cover Cuba. That said, I am not sure which countries don't have issues with Iran at the moment!

Another thought is that you use the same sort of company that high-risk workers often use (like those who were oil-industry related in the gulf wars, etc). Bellwood Prestbury is one such company, how much they would cost for an individual policy I don't know, and I don't know if they do medical only (kidnapping, evacuation, etc. may all be part and parcel of it).
