It depends on where you are talking about, as both HKG RCC and NRT RCC had showers for years and everyone can use them as long as you can get inside the lounge.

PMUA was focus more for premium pax, and that's what Arrivals by United at SFO and ORD are made for, only admit full fare int'l F, upgrade int'l F, award int'l F, full fare C, award int'l C, and all those pax must be arriving on UA metal.

There was no showers at ORD in the RCC, showers were part of the Arrivals by United only admitted by the pax I mentioned above.
