(1) What is your home airport? (SFO, LAX, MCO, etc.)
PHL is closest, but Newark or Baltimore work
(3) How many miles do you usually fly each year? (<25K, 25k-50k, >50k)
I'll probably do 18k to 26k each year. Next year might have some heavier travel, putting me closer to 40 or 50k miles. A cheap mileage run wouldn't be out of the question...
(5) What is most important to you in a FFP? (Frequent upgrades on travel, priority services when flying the airline, good award redemption rates, free lounge access, etc.)
Priority services would be nice, as would the extra checked bag.
(6) Which routes do you fly most often (US Domestic, Transpacific etc)
Its pretty much a flight to Bremen through Frankfurt or Munich every time.
(7) Preferred Airlines
United or Lufthansa, even if it adds a connection or two.
(8) Do you travel for work or pleasure? (Can you chooose your airlines, class of service?)
Economy, on my choice of airline.

I'm thinking MileagePlus might be my match, but does anyone else have input on this? EconomyPlus upgrades are very attractive.
