Ok, T-Mobile has its own corporate stores all over LA and on the webiste, there is a Store Locator, and he can check for the ones - T-Mo (authorized) Preferred Retailer - that carry the "Pay As You Go Phone Kit" locations - numerous locations, probably one that's close to the hotel he's staying at and can get to by driving over. Macy's, Kmart, Target, Walgreens, Office Depot, etc. and there's at least one in Chinatown - chances are good that he can find one that speaks the language or he's comfortable with.
All he need to ask for is to tell them he has the iPhone 4 unlocked, visiting the USA for 2 weeks and needed the $60 unlimited (everything) plan - with the proper (micro)Sim card plus a $60 prepaid/refill card. The Sim Card with activation kit is in a sealed package with long PINs or codes, ready to be activated and he will be assigned a digital mobile #.
BestBuy retail stores carry the MicroSim Card kit w. $3.34 airtime for $14.99 -

There is no activation fee for prepaid plans, the (micro)Sim card retails for $9.99 online but often waived or given free when buying a prepaid/refill card. He should spend no more than $70+tax to get this, and he can activate himself as long as he has online access and most if not all the time, the store will gladly help to do it as it will take less than 5 minutes. For address purposes, using the hotel or whatnot - no Social Security # needed and not signing a contract, no credit check or credit card required.
Call ahead to the locations that he plan to visit to make sure that they have the kit in stock to assist him and it's a worry-free process.
