1. Limit is set by your bank

3. Whichever of the two above is lowest will rule
The Indonesian bank limit is likely to be lower. For most Indonesian banks, it is Rp5 000 000 ($US500) per day. Some banks allow Rp10 000 000 ($US1000) per day; of these, the one with the most ATMs is BCA.
Most ATMs dispense Rp50 000 ($US5) or Rp100 000 ($US10) notes, up to 25 notes at a time. That would be Rp1 250 000 ($US125) and Rp2 500 000 ($US250) per transaction respectively. If there is more than one ATM of the same bank, it is likely that most are Rp50 000 and one is Rp100 000. It is usually clearly marked with a sticker. You can select English or Indonesian menu when you first insert the ATM card.
To minimise withdrawal fees, you would use an ATM that dispenses Rp100 000 banknotes and get the maximum amount: Rp2 500 000.
Please note also that moneychangers only accept $US100 banknotes in pristine condition of the newest series; no tears, marks, folds, etc. $US100 banknotes often get a better rate than smaller banknotes.
