yes...and it's sad because it does reflect upon the associate who provided the form and can create an HR issue for that associate (in the past I've informed the guest I'd give them a few days to fax me the form and I'd rebate the rate down to the appropriate rate...some times I'd receive the fax...others I wouldn't)
