It "measures" width in that if your carry-on protrudes out of it, the agent may theoretically notice and say your bag is too big. But, the general standard is "does it fit in the sizer" and since the part that measures width is open on one side, a too-wide bag will still fit in the sizer with ease. Only a bag that is too long or too deep will actually be blocked by the sides of the sizer.
