On every stay I made last year (and so far this year but only a couple of stays thus far) I recieved an upgrade to either an executive room or the equivilant/ highest at smaller properties. This was always from having booked a standard/base level room, and I was given the option for breakfast in either the lounge or restaurant.
3 exceptions...
LHR Terminal 5 - Booked standard room, was given a deluxe room and was told on check in there were no exec rooms available, they did however allow me to use the lounge regardless, don't know about breakfast options here because I only ever stay if I've got an early flight and hence get breakfast @ BA lounge.
Stockholm Slussen - Booked Deluxe room, was given a (corner) deluxe room which looked at another building... went downstairs asking if they had anything facing the water (was a holiday not business afterall), and was given an exec King room on high floor with stunning view (and therefore lounge access). You don't ask you don't get I guess, but I was particularly impressed with this because the hotel was busy (mid week stay) I had booked delux because no standards were left/ available.

And my third exception is the same as yours... Copenhagen Airport were strict on providing the one category upgrade to deluxe room... which is fine by me, I never expect anything more. They still provided a high floor room with a great view as requested, and breakfast in the restaurant was great.
I never request rooms/ e-standby/ nor-1 things in advance with the exception of Tower Bridge (often on leisure) where I normally speak to them in advance as they are great at sorting my favourite 12th floor room even though I always book standard guest room.
This June I have a stay in Milan which is the first time I have tried the Nor-1 upgrade thing for, it is currently showing I have a king exec having booked a standard guest room.
For the record yes like all I'm disappointed by the whole devlauation thing... but it's this track record and experiences above which keeps me with Hilton.
