The close-in-booking fee, only, is waived by being a Chase United MileagePlus Club cardholder. I have never had an issue having the telephone booking fee waived for domestic flights in Japan.
If I was booking from JFK-NRT with say a domestic leg to OKA I would book JFK-NRT leg online and then immediately call to have the OKA leg added. I have done this many times without fee or issue.
