Could certainly be more streamlined, I agree. I've only ever got a supplementary card during the main application process. I was told once when on the phone regarding my AMEX Gold card though that if I added a further supplementary card I would get 1000 MR points for doing so - anyone else heard about this?

(I think this thread belongs in the AMEX forum as you allude to - in fact I think we can build up a strong UK presence on the AMEX forum if we start posting pure AMEX threads there as opposed in the BAEC forum...)
