When we got married 18 years ago my wife did need to provide a copy of our marriage certificate. It took about 2 weeks for the change. More recently when the TSA required that the name on your ticket be the same as your ID she ran into the problem that her names + last names is too long for her NY Driver's licence so NY only allowed her to use her three initials plus last names but her passport has her full first name plus 2 middle names. So the name in her USAirways account were not matching the IDs she was using for trips in the US or trips abroad. To make that change she just needed to send an e-mail certifying that she was the person with that name living at this address with this telephone number and USAirways sent her an email in 24 hours aknowledging the change. At the beginning she was told to send an e-mail after her trips to get the mileage for her flights. And the miles posted in less than 24 hours after her emails.
