One other thought to avoid this problem. When you book online, on the Passenger Detail page, you can click on "I Want to Continue with my Miles & Smiles Account", fill in your M&S account number and pin, and then it will automatically fill in your name the way it is on your account. I just did it, and it automatically listed my first and middle name in the "First Name" blank. I'm assuming this may avoid the name mismatch problem.
