Log onto your GOES account and check how your name is spelled out there; GOES does have a separate first and middle field (regardless of how it appears on your Global Entry card). That is how you should have it entered on your United profile and in your Secure Flight data.
I checked the GOES account and the fields match in terms of First, Middle, Last. Date format is different but all the numbers match. It all seems hopeless.
