Have had something similar happen to me before (whilst exiting a non-EU country returning to LHR). Had to ask 3 different people in order to get an exit stamp on my visa.

These kinds of things can cause real problems for people when they return to the same country the next time.

This incident seems more like an oversight unlike my example which was just laziness.
