I'm on a flight this weekend LAX -> JFK (part of an international itinerary) and on a full Y fare. BE is 100% full, no chance for upgrade for medallion or SWU.

So yeah, it happens. But generally I fly this route 2-3 times per month and there's normally at least one BE seat open.
