Anything less than MVP Gold, and that would only be true for full fare (ie non-saver) awards.
That rule went away some time ago (
missy announced it). Now, you pay any difference in fare for any changes to any legs of flight, no exceptions (except the Same Day Change exception, which is mostly useless).
