For 1-3, how many guests can I bring if I don't have a spouse? Would they also have to pay?
I'm assuming you have a US-issued card (rules vary depending on which country issued your card):

1) Any of the above, except for United Club
2) Any of the above, except for United Club
3) Any of the above, except for United Club. Also, you can access any Admirals Club lounges in LAX.

You can bring in two travel companions for US Airways, American Airways, and Delta lounges, provided you are flying with them (except for US Airways, which you can access without flying with them). If you have to use your Priority Pass to access a lounge, there is no free guest - it's $27 for each guest, including spouse.
