I just completed the interview about a month ago at the Ambassador Bridge in Detroit. From what I've seen on FT, the questions and process varies slightly from location to location. Here's what happened to me in regards to your questions:
1) They did ask me to name all the countries I'd been to (verbally). The agent looked like she was checking them off the list on the screen. I don't think I got all of them, but the agent basically just moved on to the next question. Also, I hadn't been to any questionable countries (primarily Caribbean and Europe).
2) I was required to give my driver's license at the interview. My driver's license had the wrong address but there was no mention that it didn't match what was on the application. I brought along a copy of my mortgage, insurance, and a recent utility bill just to be sure. The agent didn't ask to see them.
