Then why have I never been charged any foreign transaction fee when making purchases in Cambodia? That the default currency is USD is crux (as my cards as US based), but it's also dependent upon the bank's policies. Last I looked Cambodia was an independent country.
That said, you're better off paying the FOREX fee, if the card in use charges it, than to pay DCC (moreso if you're going to be dinged for a transaction in a foreign country on top of it).
