You will probably get the best rate by taking money from an ATM. Other than that, changing USD in Hong Kong would be best - it is almost a fixed rate, so you should be able to work out what is good value easily.

Generally the lowest table has a minimum of HK$200 (around US$25) in the larger casinos, but there are only a few of these. Many tables are HK$300. You might get HK$100 tables during the day if you are very lucky (I really haven't seen these for a while), but smaller more chinese orientated casinos may be a better bet for the cheaper tables.

The chinese are generally hardcore gamblers - a very different vibe in the casinos as compared to Las Vegas - so it is not unusual to see some HK$10,000 bets at ordinary tables.
