I recall flying ORD-LHR in F last year during which I asked for Dine on Request as soon as the steward came along to take orders. I was told I could have my meal with everyone else or not at all. There wasn't any reason given for not offering Dine on Request (which was listed as an option in the menu) and the fellow was so grumpy that I didn't push the issue.
Typical example of AA lacking control of their inflight premium product. At some point management is going to have to figure out a way to actually execute what they advertise. No premium foreign carrier would let this happen - if it is part of the service, it will be offered by the crew.
