1. By Silver equivalent, I presume you mean the CX status that is equivalent to Silver? This is an official benefit for the main cardholder only, not any supplementary cardholders (though there have been reports of a few supp. cardholders getting approved too). To ensure both of you get the Silver-equivalent status, you'd both have to get the Plat. card. If you do this, you should get one first and then, once approved, refer the other so you get the two sets of bonuses.
2. Its easy to cancel the card, but the Quidco cashback can take more than 3 months to come through from Amex, so there is a danger you won't get it if you cancel in 3 months. I'm still waiting for my cashback for the Gold card I got back in June - its Confirmed, but not yet Payable.
3. Yes, the £450 fee is charged in your first month statement so is payable then. How quickly you get the refund depends on how its paid back. If you have another card, Amex can transfer it to that (though there are some reports they can't do Charge Card -> Credit Card credit transfers) and it'll credit within a day or so. If you need them to send you a cheque, that'll take longer - I'd wager on a few weeks.
4. I can't see an end date on the T&Cs but it does say the offer can be withdrawn at any time, so I guess you have to weight up the risks. Its unlikely to disappear, but the current 50K is a good bonus.
