Rare, sure, but consider how many passengers enplane every year.
My sister (22 at the time) had DVT on a JNB-LHR flight, and she is otherwise extremely healthy (an athlete). Apparently the issue was fairly severe, and the doc said that she was lucky that she got ER attention upon landing.
