Something similar happened to me two years ago. Using UA miles, I had booked award travel on LH but broke my hip and was unable to fly. When I phoned to cancel the flight, I was told that the miles (60k) would be redeposited for a fee ($100 or $150, I think), but that the fee would be refunded if I mailed in a written explanation with a note from the doctor. Everything was handled very smoothly. The miles were redeposited in a day or two, and the refund was processed within a month or so.
