Remember that on many (most?) non-US airlines it is certainly NOT a given that business class will be filled with upgraded elite members of the FF programme. Upgrading is not completely unheard of, but I have often flown on SA with empty seats in C. The idea is that you pay for what you get and the value of the product is not diminished by giving it away as upgrades. Maybe once in a while to real high value customers. Op-up due to an overbooked Y cabin does happen.
