I had the hotel arrange this and charge to room it was $40 or $50 which is quite expensive however after waiting over an hour at immigration I wasn't out until near midnight and the drive was about 45 mins -easy to see name/reservation on the board and get driver. Nice to know he called ahead so front desk was ready for me. You could look to book direct w/company or get local for less than $20 -cabs were tricky IMO and traffic a game of frogger. if you DM me I do have a taxi driver I met who I would use again you can contact him for rates.
Hotel was great, service by concierge and restaurant (esp breakfast staff). Shuttle to Miraflores waterfront mall and lots of taxis out front.
