The UA232 incident led to check valves being placed in the hydraulics to prevent fluid loss in the event of system damage to all three redundant systems. IIRC the probability of all three systems failing was ROM 10E-9.....

Also noteworthy on 232 was an incredible piece of flying, including controlling all three degrees of freedom with only differential thrust on two engines on the DC10.....
