I always recommend Etymotic Research. I don't have any that have a mic or volume control - I have the HF5 - but they make some that do. I believe the MC3 would be in your price range with what you want.

I can tell you that the Etys are the best sounding earbuds I've ever used and do a great job blocking noise.
