My mom (married to Italian citizen) has had a couple of these (they gave her 2 years each time), but they were issued by the Italian embassy, so it wasn't difficult.
We also asked about it at the Spanish consulate and they said that it was important to have the Italian marriage certificate... You might need the marriage certificate registered at your country of citizenship.
