My bad for using the term "instant upgrade" - I should have realized that might confuse them. I did offer up the fare code she balked at it - didn't care.

No, they were combinable and even said so in the fare rules (not that this was what the agent was looking at).

That is basically what I ended up doing. Booked a roundtrip AAA-BBB-AAA outbound in the instant upgrade class, return in coach. Had them change it to AAA-BBB-CCC (instant upgrade outbound, coach return).
Worked out in the end but what a PITA.
