Those are two more worry points, IMO. Time to call AA (again!) and add some verbal laxative to this process.
(As noted below, things are pretty hectic due to Sandy right now and it might pay to call later this week.)

I actually called BA a few days ago to put in a meal request. BA is the last leg back to the UK (when booking, virtually all available itineraries had this). BA had all the details of the trip including the preceding connecting AA flight, but could not do the meal request or look up seating, as it had not yet been ticketed.
I have now discovered that my credit card has not been debited either.
