I would beg to differ slightly... had one leg of a flight on United that they would not credit miles for even though I appealed it twice :-( Their reason was that it was not a correct fare class for collecting miles.

And I had one UA flight that was not credited because "the aircraft you flew was not eligible." Not flight number or fare class (which was valid) but aircraft (!). Totally ridiculous but true. I gave up fighting it because it was only 1000 miles.
