Military acft - KC-135 certified to 50k ft. The only time it makes (fuel efficiency) sense to climb there is if you're lightweight, are already at altitude, and have a couple hours of cruise left. Because few pilots in the community ever find themselves in this scenario, it's a pretty rare event. I've heard that the jet performs like a complete dog, but (barely) seeing the curvature of the earth is something special.
