I believe Jetsgo is also e-ticket only systemwide. And they start serving FLL on Dec. 21, 2002, making them an international carrier.

Heck, I think even Air Canada's own, Tango, supposedly a separate carrier, is e-ticket only and they fly to LAS starting December 26.
