Can't get the link to work.
I have tried to use this cut and paste link

The problem is that when this link opens, there is no way I can change the name John Smith to my name nor can I change the member ship number 22222222.

It would allow me to enter my information however when I go to verify page, it shows the name as John Smith. Any help would be appreciated.
