I've seen this neatly addressed at a cellphone store in DOK - they offered to cut, and also to sand/slim down properly the prepaid sim, that they sold me - this came with a promise, that if it gets stuck, it's their responsibility to remove, or give me a new phone (they actually had unlocked 4S in stock, so it was semi-believable). At any rate, the guy who cut the sim spent some time with a hobby knife perfecting and shaving that sim, put it in, and then took it out - I've used it on 3 trips now.
I have a card cutter for micro, but I'm not touching it anymore.
If anything, I'd rather get a local store to do the cutting to micro or nano, and show me, that it comes out
